It is very important to preserve medical records for use at a later date.
Well kept records come handy at times when you need them.
Medical records include:
1.Patient ID given by the hospital.
This is required to retrieve the records at the Hospital
and also to keep track of various departments who provided medical
treatment and tests and when.
2.Prescriptions\Case sheets\Discharge summaries.
3.ECG,X Rays,CT Scan,Angiogram,Ultrasound scan,
Kidney Stone Analysis,Blood,Urine,stool,
Biopsy report, Blood grouping,Allergy test,Thread mill test and others.
Take a xerox of ECG (if the original is on a Thermal paper).Do not fold X
rays.Most other reports are available in Electronic format.
Keep records in digital format.Check with hospital about the software
in which the report can be read.
4.Insurance Policy \Cash less treatment Card issued by the Insurance coy.
5.List of hospitals where you can avail treatment and their
phone numbers,address and location map
name and phone no of Insurance co ordinator located at the hospital.
6.Open a seperate file and keep the papers in date order.Also keep
the files in your active e mail id so that you can locate it from any
where in the world.
7.Most importantly check with the hospital about their records
preservation policy.
Ask them as to for how many years they preserve patient records and
in what form - physical \digital.
